Preschool Pre-Registration

If you are interesting enrolling for the 2022-2022 school year, please fill out this pre-enrollment form and someone will contact you: 
 
 
 
STATE FUNDED PROGRAM
 
Once we contact you, we will schedule an intake appointment. The following items will be required for those applying for State Funded Preschool at the intake appointment:
 
 Birth Certificates for every child in the family. Preschool child must be 3-4yrs. of age
 Copies of your most recent pay check stubs for a one-month period. These stubs are necessary to determine eligibility in the State Funded Preschool Program. If both parents are working we will need monthly pay stubs from both parents. If self-employed – a Bank Statement is needed, plus Business Card, Business Advertisement, Business Tax ID #, and Business License (if applicable). If income is in the form of public assistance, copy of the most recent Notice of Action from AFDC needs to be sent as it includes income and Medi-Cal number.
 Copy of your child’s immunization records (Little Yellow Booklet)
 Proof of residence indicating that the parent is the responsible party: lease or rental agreement.
 Physical form filled out by the child’s primary doctor.
 Documentation that confirms the presence or absence of a parent (only required for single family household).
